Each Application qualified college student is encouraged to apply. Applicant must have previously applied for and received an Initial DPSOA Scholarship as a graduating high school senior or other eligible applicant. Application for a Continuing Education Scholarship must be made within four years of receiving an Initial DPSOA Scholarship. Applicant must have successfully completed a minimum of 24 college semester credit hours within a consecutive 12 month period and have a minimum cumulative grade point average of 2.75 on a 4.00 point grading system. A maximum of three Continuing Education Scholarships may be received within the four year period. Awarded scholarship checks will be sent to the applicant and made payable to the college, university, or trade school of the applicant’s choice. The granting or non-granting of a scholarship will be at the discretion of the Scholarship Committee. A $25 fee will be charged for checks necessitating reissue.

APPLICATIONS MUST BE POSTMARKED BY THE UNITED STATE POST OFFICE
NO LATER THAN JUNE 15 OF THE CURRENT YEAR. Applicant must initial on this form each item submitted and include this form with the application for scholarship.
